What Is the NHTD Program?

The Nursing Home Transition and Diversion (NHTD) Medicaid Waiver Program is funded through Medicaid in New York State. It is designed for seniors and individuals with physical disabilities who qualify for nursing home level of care but prefer to live in their community.

The program provides services that support independence, personal choice, and community integration.

Who Is Eligible for NHTD?

Eligibility criteria

You may qualify for NHTD services if you:

  • Are 18–64 with a physical disability or 65 and older
  • Are eligible for Medicaid
  • Require nursing home level of care
  • Prefer to live in the community instead of a nursing facility
